To have your most recent examination records released to Eye Carumba Optometry, download the "Request for Release of Healthcare Information" form, fill in the fax coversheet with your name and phone number, complete the Release form, and fax the coversheet to Eye Carumba Optometry (fax number indicated on form).

Streamlining Your Paperwork
Upon arrival for your appointment, all patients are requested to complete the Patient & Health History Form and the HIPAA Authorization Form. Due to the confidential nature of these forms, similar forms completed at Dr. Torres' previous practice were not transferred to this office. You are encouraged to download these forms, complete them, and bring them with you to your appointment.
