For routine eye exams (wellness care) and primary eyecare (treatment of medical conditions of the eye) Eye Carumba Optometry is an in-network provider for Vision Service Plan (VSP) and the Blue Shield PPO insurance plan.  For primary eyecare, Eye Carumba Optometry is also an in-network provider for Blue Cross PPO insurance plans.

For other insurance plans, our office staff will provide a detailed receipt at the time of payment, including diagnostic and treatment codes that can be submitted to other insurance carriers for possible out-of-network reimbursement.

The Eye Carumba staff will gladly assist patients in determining current eligibility by contacting VSP, Blue Shield, and Blue Cross directly when necessary.  However, it is the responsibility of the patient to know and understand applicable benefits and co-payments.  Patients are responsible for full payment of any charges refused by the appropiate insurance carrier.
