If you are a new patient, complete the Patient Intake Form and the HIPAA Authorization Form. If you plan to fill the forms out in our office, please come in 15 minutes early to handle the paperwork. (New Patients Only)

Your eye examination appointment with the doctor is scheduled for 45 minutes. Please allow 5 to 10 minutes for check out.

Your eyes may be dilated as a part of your eye examination. If dilation is necessary, expect for your vision to be blurry up close and light sensitive for up to 3 hours.

Come in wearing your contact lenses for at least 1 hour.

If you are a new patient, please bring the contact lens packaging from your contacts or a copy of your last contact lens prescription. (New Patients Only)

We ship annual supplies of contact lenses direct to you free of charge.

Bring all prescription glasses that you currently use including readers and sunglasses.
